Adding a care questionaire to the health section

I know a couple of our members from another gecko forum *ahem* you know the one Anywho... recently a "care questioaire" was added as a sticky to their health section to be filled out by posters who have questions about their leo's health. The reason for this care questionaire was to have a good idea of their basic husbandry and leo's history so the forum membership isn't playing 20 questions just to find out if the op's leo is sick because of bad husbandry.

It would include things like age, morph, gender, living situation, tank size, substrait, heating, feeding schedule, suppliments/how often, etc. These are ALL basic questions we ask time and again to find the root issue involved in any depressed leo. I have seen/used a form like this on other forums that I frequent and think it saves the membership a lot of time and frustration when we can see ALL the op's husbandry techniques.

I would be happy to write something up if y'all think it would be helpful.

I think that would be a great idea!!

It would save having to ask a member with a herp health problem a million questions to try to piece togethers what's wrong.

It would also allow them to get a quick answer on what might be wrong with their leo.. and allow them to get medicine or other stuff before it's too late.

Haha i was wondering when this was going to pop up lol. Yeah i'd be happy for you to do it Kel, i mean its a great idea, i just didn't want to steal 'Other forums' idea's too soon lol.
But by all means go for it.

Once you've written it, i shall try and find some way, of including a notice within the 'writing a new thread' page to mention 'please fill out the questionnaire before posting' . 'try' being the key word here though.

Yeah well even on the "other" forum it actually WAS my idea but some one that I was co-collaberating with took my idea once it got approval, put their name on it and wrote it up.

I'll get on it soon. Just make it as a sticky... you'll see.
